AIDS Council of Northeastern New York

The mission of the alliance for positive health is to reduce the impact and incidence of HIV/AIDS and other serious medical and social conditions in northeastern NY.

Health home services: through the agency’s health home program 980 clients were provided 11,340 care coordination services. Under these programs, agency staff working in collaboration with other community services providers connect clients to health care providers, mental health, and substance use providers. They also support client’s in accessing medication resources, housing assistance, social services such as addressing food insecurity, need for benefits, and transportation, and any other services that help ensure clients’ wellbeing.
