Empowering communities towards health and wellness by championing the cause of those at imminent risk and the homeless.

The organization provided housing navigation services to homeless individuals and those at risk of homelessness, including resource navigation, education, and prevention and diversion assistance. In collaboration with local housing and homelessness agencies, the organization utilized the Homeless Management Information System (HMIS) to efficiently allocate funds and provided financial assistance for housing deposits and essential move-in needs.
